Vial of Life

The Vial of L.I.F.E. is Lifesaving Information For Emergencies.  It is a place for you to store important medical information that emergency medical personnel (firefighters, paramedics, hospital staff) need in a time of crisis. 

By completing the Vial of L.I.F.E. form, your provide the emergency personnel with vital lifesaving information,even if you are unable.  It is very important the you keep this information up to date, accurate, and placed in a prominent place in your refrigerator.

The Vial of L.I.F.E. Kit is available free as a public service from the Union City Fire Department. A kit may be obtained by contacting Fire Station #1 at 510-675-5471 during normal business hours, or the Fire Department Emergency Medical Services Division at 510-675-5429.

How to use the Vial of L.I.F.E.

• Complete the Vial of L.I.F.E. form. Be sure to include the date that you completed this form (this will help emergency personnel know how recent the medical information is).
• Fold the Vial of L.I.F.E. form and put it inside the Vial of L.I.F.E. container.
• Place the Vial of L.I.F.E. container on the top shelf inside your refrigerator door, closest to the door handle.
• Attach the Vial of L.I.F.E. sticker to the outside top corner of your refrigerator door to alert emergency personnel to the presence of Vial of L.I.F.E. information.
